ABSTRACT

Knee osteoarthritis is one of the common type of arthropathy, the clinical stage of the typical patients belongs to the middle-late stage, so it urges to improve the early diagnosis. At present, magnetic resonance imaging is most used in clinical diagnosis of knee osteoarthritis, and with the development of different MRI sequences, the sequences of early articular cartilage lesions are used in clinic. In the early diagnosis of knee osteoarthritis, the simple and practical methods such as ultrasonography is becoming a trend, and the specific biomarkers of early knee osteoarthritis have become the hot research. This overview article outlined the methods of early diagnosis from the ultrashort echo time MRI, ultrasonography and biomarkers.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the diagnostic value of the radiologic characteristics of osteoporotic Kummell's disease.</p><p><b>METHODS</b>Total 16 patients with pathologically confirmed osteoporotic Kummell's diseases were reviewed from May 2010 to May 2012, including 4 males and 12 females with the mean age of 73.4 years (ranged, 67 to 83 years old). Radiologic imagings of all patients, including X-ray, CT and MRI, were analyzed retrospectively.</p><p><b>RESULTS</b>Intravertebral linear clefts could be seen on the AP and lateral X-ray films of vertebrae. Sagittal and axial CT scans demonstrated the vacuum cleft phenomenon with liquid and air was identified within the vertebral body. Sagittal MRI showed the callapsed vertebral segment and the area of fluid signal with clear and intact border within the vertebral body. The fluid signal was low on T1-weighted images and high on T2-weighted images and stir images, which was corresponding to an intravertebral vacuum cleft.</p><p><b>CONCLUSION</b>The radiologic characteristics of Kurmmell's diseases can provide valuable evidences for the early diagnosis.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe effects of serum Bushen Huoxue prescription(Chinese characters) on classic Wnt/β-catenin signaling pathways of osteoblasts, and explore mechanism of Bushen Huoxue prescription (Chinese characters) for preventing osteoporosis.</p><p><b>METHODS</b>Twenty health female rats were randomly divided into two groups, including Bushen Huoxue (Chinese characters) group and saline group,10 in each group. Bushen Huoxue (Chinese characters) group and Saline group were gavaged Bushen Huoxue and saline every day for 1 week. Bushenhuoxue containing serum and saline containing serum were got according to methods of serum preparation of drug-containing. The osteoblasts was cultured with neonatal rat skull according to Enzyme Consumer Law, and was identified by Wright-Giemsa staining (R-J) and alkaline phosphatase staining (ALP). The third generation of osteoblasts was divided into three groups, including saline group, normal group,Bushen Huoxue (Chinese characters) group. Each group were added to 15% appropriate medium. ALP activity of osteoblasts and osteoblasts proliferation rate were tested, mineralized nodules were observed, the expression of β-catenin, Runx2, Osx mRNA of osteoblasts were tested by RT-PCR.</p><p><b>RESULTS</b>There was blue granules in cytoplasm, cell nucleus was flint with 1 to 3 nucleoli showed by R-J staining, morphology of osteoblasts were cultured. ALP staining showed cytoplasm with purple granules, the results showed that the cultured cell was osteoblasts. The content of ALP in Bushen Huoxue (Chinese characters)group was (6.272±0.131) ,appreciation rate was (0.81? 0.172), and could significantly improve differentiation and proliferation activity of osteoblasts compared with Saline group (P< 0.01). There were four different size orange nodules, the Maximun nodule was 1.0 x 1.0 cm in Bushen Huoxue (Chinese characters) group after Alizarin red staining, the results showed Bushen Huoxue (Chinese characters)group could obviously improve mineralization of osteoblasts. The expression of mRNA of β-catenin, Runx2 and Osx in Bushen Huoxue (Chinese characters)group were (1.782±0.944), (1.935±0.994) and (1.610±0.811) by RT-PCR,it was significantly increased compared with saline group (P<0.01), but there was no difference between Bushen Huoxue (Chinese characters)group and normal group (P>0.05).</p><p><b>CONCLUSION</b>Bushen Huoxue (Chinese characters)group could obviously promote differentiation, proliferation and mineralization of osteoblasts through activation of Wnt, β-catenin signaling pathway. It suggested that the mechanism of action of Bushen Huoxue (O'f f Il.t)particle clould prevent osteoporosis through the activation of Wnt, β-catenin signaling pathway.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To analysis the effects of restoring neck length and femoral calcar of femoral component on the reconstruction of femoral offset and its affection on soft tissue balance in total hip arthroplasty with standard femoral components.</p><p><b>METHODS</b>Sixty-eight patients (71 hips, 30 male and 38 female,ranging in age from 17 to 89 years,with an average of 63.5 years)undergone primary total hip arthroplasty were followed up. The postoperative hip scored according to Harris hip score symtem, and femoral offset,femoral calcar restored as well as neck length of femoral component were measured from the postoperative X-ray image, and the adductor muscle force was measured by Cybex-6000 isokinetic muscle strength testing system.</p><p><b>RESULTS</b>Femoral neck length had highly significant and positive correlation with femoral offset (r = 0.64, P < 0.001), and had more effects on femoral offset than restoring femoral calcar (t = 3.07, P < 0.01), but had no significant difference on postoperative Harris score. The postoperative femoral offset of patients operated for the femoral head osteonecrosis was less than those for the hip osteoarthrosis (t = 2.91, P < 0.01) and the femoral neck fracture (t = 3.31, P < 0.01). The adductor muscle force in patients who restored femoral offset were better than those failed to restore femoral offset.</p><p><b>CONCLUSION</b>There are several factors affecting femoral offset restoration for soft tissue balance in total hip arthroplasty with standard femoral components, among them, the key factor is the neck length of femoral component which is more significant than femoral calar restoration for reconstruction of femoral offset. The role of femoral offset on soft tissue balance in total hip arthroplasty was strengthening the postoperative adductor muscle.</p>
